Drug-related problems (DRPs) become a major cause admission these units, as well high rate short-term readmissions. Anticoagulants, antibiotics, antidiabetics, opioids been shown be most common drugs involved this issue. Inappropriate has pointed out one causes visits. Different ways conducting chronic medication reviews at discharge, primary care coordination, phone contact discharge reduce new hospitalizations room visits due DRPs, they are key elements for improving quality provided by services.
