Introduction Special Operations Forces (SOF) often conduct operations in physiologically stressful environments such as severe heat, cold, or hypoxia, which can induce decreases a variety of cognitive abilities. Given the promising empirical demonstration efficacy exogenous ketone monoester (KME) supplementation attenuating performance decrease during hypoxia at rest laboratory setting, we conducted real-world, field experiment examining KME’s high-altitude mountaineering, an austere environment US SOF have increasing numbers over past two decades. Methods Specifically, 34 students and cadre Army 10th Group Advanced Mountaineering School (SOAMS) participated randomized, double-blind, placebo (PLA)–controlled crossover trial (KME vs. PLA) 2 days tactical mountain training. The participants ascended from 7,500 ft altitude (basecamp) to 12,460 on 1 day 13,627 other (in randomized order), while performing various training activities inducing high physical loads 8–12 h, consumed six doses KME PLA 2–3 h apart throughout each day. Results Discussion While increased blood levels decreased glucose levels, there were no clear indications that elevated level enhanced performance. also produced greater incidence heartburn, nausea, vomiting. In these elite operators, mountaineering had limited impact performance, did not demonstrate any benefit.

Abstract Increasing evidence suggests cardiac function improves in healthy and failing hearts alongside circulating ketones (1–4 mM). This study characterized blood flow responses to a ketogenic beverage compared volume/calorie matched placebo with repeated imaging over 120 min. was two‐group, placebo‐controlled, acute study. Adults without abnormalities underwent baseline MRI including quantitative myocardial perfusion measure (MBF). Subjects consumed 50 g of ketogenic‐promoting [bis‐hexanoyl R‐1‐3‐butanediol (BH‐BD)] (BH‐BD; n = 11) or calorically/volume‐matched lipid‐based (PL; 10) every 15–30 Following min, subjects final scan MBF measurement. R‐BHB glucose were measured at timepoint. min following BH‐BD consumption, reached 2.1 mM. Cardiac output (CO) elevated PL ( p < 0.05) increased +31% after ingestion 0.001). Introduction: Bis-hexanoyl (R)-1,3-butanediol (BH-BD) is a novel ketone ester that, when consumed, hydrolyzed into hexanoic acid (HEX) and (BDO) which are subsequently metabolized beta-hydroxybutyrate (BHB). Methods: We undertook randomized, parallel, open-label study in healthy adults ( n = 33) to elucidate blood BHB, HEX BDO concentrations for 8 h following consumption of three different serving sizes (SS) BH-BD (12.5, 25 50 g/day) before (Day 0) after 7 days daily 7). Results: Maximal concentration area under the curve all metabolites increased proportionally SS were greatest BHB followed by then on both Day 0 7. Metabolite half-life tended decrease with increasing HEX. Time peak days. In vitro incubation human plasma demonstrated undergoes rapid spontaneous hydrolysis. Conclusion: These results demonstrate that orally ingested products appear undergo conversion dependent manner, metabolism neither becomes saturated at up g nor displays consistent adaptation consumption.
