Infertility affects 8–12% of couples worldwide, and 30–75% preclinical pregnancy losses are due to a failure during the implantation process. Exposure endocrine disruptors, like bisphenols, among others, has been associated with increase in infertility observed past decades. An correlated exposure disruptors bisphenols. The healthy status uterine microorganisms, predominantly Lactobacillus, plays a crucial role in reproductive health. Studies have shown that composition changes with hormonal fluctuations during menstrual cycle, affecting receptivity embryo implantation. presence specific bacteria, such as Lactobacillus species, has been associated successful pregnancy outcomes, while dysbiosis linked to implantation failure infertility. Visceral fat accumulation and dyslipidaemia are associated with infertility symptoms. The cardiometabolic index (CMI) is a comprehensive quantitative measure of central obesity dyslipidaemia. However, the link between female CMI couple needs to be explored further. Hence, this study aimed dissect connection. used cross-sectional approach select 1,641 participants from National Health Nutrition Examination Survey (NHANES), which was conducted 2013 2020. natural log-transformed (In-CMI) consider non-normal distribution CMI. After adjusting all covariates, each 1 unit increase in 34% incidence (odds ratio [OR] 1.34, 95% confidence interval [CI] 1.10-1.64, p = 0.004). In addition, remained statistically significant after dividing into tertiles (T1, T2, T3). T3 group, highest In-CMI, showed higher odds compared T1 group (OR 2.11, CI 1.38-3.23, < 0.001). Infertility is a widespread medical condition, affecting notable percentage of couples globally, with rising prevalence in India. While assisted reproductive technologies (ART) provide hope, recurrent implantation failure (RIF) continues to challenge approximately one 10 undergoing in-vitro fertilization (IVF). This expert opinion document intends highlight the promise probiotics, particularly from Lactobacillus, as therapeutic approach for improving fertility and treating RIF. In India, on June 29-30, 2024, physical meeting was organized involving 14 specialists gynecology, obstetrics, fields. The role probiotics health discussed an emphasis infertility All experts agree that microbial dysbiosis characterized by imbalance among dominant Lactobacillus species associated RIF leading increased inflammation hence poor outcomes. Probiotics have emerged possible therapies restore equilibrium while reducing enhancing intactness genital epithelium barrier. strongly supported use Lactobacillus-based oral probiotic supplementation preventing miscarriages maintaining pregnancy. unanimously agreed supplementation, Lactobacillus-based, should be considered potential method prevent aid They recommend prior embryo transfer enhance pregnancy rates. emphasizes value safe non-invasive By fostering balanced microbiome, may increase likelihood successful pregnancies.
