Objectives: This study aimed to evaluate the imaging and clinical characteristics of adrenal metastases detected by [68Ga]Ga-PSMA PET/CT in prostate cancer patients, with a focus on diagnostic accuracy prognostic implications. Specifically, we examined correlation between lesion markers, such as prostate-specific antigen (PSA) levels Gleason scores. also assessed performance PSA, standardized uptake value maximum (SUVmax), Hounsfield Unit (HU) values differentiating from benign adenomas. Materials Methods: retrospective included 44 patients lesions identified using January 2020 October 2024. The were categorized into two groups: adenomas (n = 16) 28). was performed 5-ring Discovery IQ scanner QClear reconstruction, following injection 2.5 MBq/kg ligand time 60 min. parameters (SUVmax HU values), (PSA levels, scores, presence lymphadenopathy), patient outcomes analyzed. A ROC analysis conducted these key parameters. Results: Patients had significantly higher PSA (mean: 45.6 ± 12.4 ng/mL vs. 18.3 6.7 ng/mL; p < 0.01) scores (median: 8 6; than those SUVmax elevated metastatic 12.8 4.3 3.4 1.2; 0.001), 45 15 18 10; 0.01). revealed that highest (AUC: 0.87), followed 0.85) 0.80). Disease progression observed 67.9% cases versus 18.8% adenoma group (p median overall survival shorter (24 months 38 months; Conclusions: is valuable modality for distinguishing patients. integration SUVmax, workflows enhances precision improves decision-making. Future research should prospective validation findings larger cohorts explore artificial intelligence-based approaches automated characterization.
