Using data from the 2022 and 2023 National Survey on Drug Use Health, we examined factors associated with treatment use for substance disorder (SUD), perceived SUD needs, reasons non-use. Of U.S. adults, 18.1% had any past-year (alcohol [AUD] and/or drug [DUD]), 14.4% of those received in past year, 5.5% who did not receive a need treatment. Treatment was significantly AUD DUD severities (aOR = 3.85, 95% CI 2.82–5.26 severe AUD; aOR 2.82, 2.27–3.47 DUD), problem self-perception 2.12, 1.74–2.58), mental health 6.07, 4.73–7.78). Perceived needs among were also severities, self-perception, illness. The most frequently reported non-use self-sufficiency beliefs, lack readiness to stop using or start treatment, stigma-related concerns, insurance/cost problems. findings underscore importance screening educating about harms untreated increasing motivation people SUD.
