To describe the placental pathology, fetal autopsy findings and clinical characteristics of pregnancies that resulted in stillbirth owing to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) placentitis, identify potential risk factors.This was a prospective multicenter study non-vaccinated pregnant women affected by disease 2019 (COVID-19) Greece from April 2020 August 2021. A total 165 placentas were examined histologically six cases associated with SARS-CoV-2 placentitis retrieved. Complete performed three these cases. Gross, histopathological, immunohistochemical, molecular electron microscopy examinations carried out organs. The histological compared those 159 maternal COVID-19 which live birth. Regression analysis used predisposing factors for placentitis.The all stillborn showed extensive changes typical characterized combination marked intervillositis mixed inflammatory infiltrate massive perivillous fibrinoid deposition trophoblast damage, intensely positive immunostaining spike protein, presence virions on reverse-transcription polymerase chain reaction test tissues. lesions obliterated over 75% intervillous space, accounting intrauterine death. Similar affecting less than 25% placenta observed seven liveborn neonates, while remaining 152 COVID-19-affected birth did not show findings. evidence an asphyctic mode death without viral transmission fetus. mothers had mild symptoms or asymptomatic, interval between diagnosis ranged 3 15 days. Statistically significant included thrombophilia prenatally diagnosed growth restriction (FGR). Multiple sclerosis seen one case.SARS-CoV-2 occurred uncommonly and, when extensive, caused demise, no transplacental infection. Thrombophilia detected FGR emerged as independent potentially lethal placentitis. © 2022 International Society Ultrasound Obstetrics Gynecology.

Abstract SARS-CoV-2 infection causes COVID-19. Several clinical reports have linked COVID-19 during pregnancy to negative birth outcomes and placentitis. However, the pathophysiological mechanisms underpinning placentation early are not clear. Here, shed light on this, we used induced trophoblast stem cells generate an in vitro placenta model. We identified that syncytiotrophoblasts could be infected through angiotensin-converting enzyme 2 (ACE2). Using a co-culture model of vertical transmission, confirmed ability virus infect previous endometrial cell infection. further demonstrated transcriptional changes led impairment cellular processes, reduced secretion HCG hormone morphological vital for syncytiotrophoblast function. Furthermore, different antibody strategies antiviral drugs restore these impairments.
