Acute pulmonary edema is a leading cause of death in patients with preeclampsia. The authors reported case pregnant woman at 25 weeks gestation severe preeclampsia complicated by edema, who required an emergency cesarean section, posing significant challenge to the anesthesiologist. patient had developed Type 1 respiratory failure and needed supplemental oxygen high-flow nasal oxygen. Due contraindications for neuraxial anesthesia, section was performed under general anesthesia. After induction patient’s hypoxemia worsened. Eventually, after treatment fluid restriction, diuretics, albumin, oxygenation improved gradually, procedure successfully. Both newborn good prognosis.
