General anesthesia produces multiple side effects. Notably, it temporarily impairs gastrointestinal motility following surgery and causes the so-called postoperative ileus (POI), a multifactorial complex condition that develops secondary to neuromuscular failure mainly affects small intestine. There are currently limited medication options for POI, reflecting lack of comprehensive understanding mechanisms involved in this condition. although acetylcholine is one major neurotransmitters initiating excitation-contraction coupling gut, cholinergic stimulation by prokinetic drugs not very efficient case POI. Acetylcholine when released from excitatory motoneurones enteric nervous system binds activates M2 M3 types muscarinic receptors smooth muscle myocytes. Downstream these G protein-coupled receptors, cation TRPC4 channels act as focal point receptor-mediated signal integration, causing membrane depolarisation accompanied action potential discharge calcium influx via L-type Ca2+ myocyte contraction. We have recently found both inhalation (isoflurane) intravenous (ketamine) anesthetics significantly inhibit current (termed mICAT) ileal myocytes, even proteins activated directly intracellular GTPγS, i.e., bypassing receptors. Here we aim summarize Transient Receptor Potential signalling-related aspects gut visceral pain, discuss exactly how may be negatively impacted general anaesthetics, while proposing receptor-operated channel novel molecular target treatment
