Extracellular particles (EPs), including extracellular vesicles (EVs) and non-vesicular (NVEPs), are multimolecular biomaterials released by cells that play a crucial role in intercellular communication. Recently, new subtypes of EPs associated with central nervous system (CNS), such as exophers supermeres have been identified. These provide perspectives for understanding the pathological progression CNS disorders confer potential diagnostic value liquid biopsies neurodegenerative diseases (NDs). Moreover, emerged promising drug delivery vehicles targeted platforms CNS-specific therapies. In this review, we delineate landscape EP their roles pathophysiology diseases. We also review recent advances EP-based diagnosis NDs highlight importance analytical single-particle resolution exploitation biomarkers. Furthermore, summarize application engineered EVs treatment outline underexplored NVEPs novel therapeutic agents.

Intervertebral disc degeneration (IVDD) is a prevalent condition contributing to various spinal disorders, posing significant global health burden. Mitophagy plays crucial role in maintaining mitochondrial quantity and quality closely associated with the onset progression of IVDD. Well-documented region-specific mitophagy mechanisms IVDD are guiding development therapeutic strategies. In nucleus pulposus (NP), impaired mitochondria lead apoptosis, oxidative stress, senescence, extracellular matrix degradation synthesis, excessive autophagy, inflammation, instability, pyroptosis, key regulatory targets including AMPK, PGC-1α, SIRT1, SIRT3, Progerin, p65, Mfn2, FOXO3, NDUFA4L2, SLC39A7, ITGα5/β1, Nrf2, NLRP3 inflammasome. annulus fibrosus (AF), damage induces apoptosis stress mediated by while cartilage endplate (CEP), dysfunction similarly triggers stress. These mechanistic insights highlight strategies such as activating Parkin-dependent Ub-independent pathways for NP, enhancing AF, targeting Parkin-mediated CEP. include use natural ingredients, hormonal modulation, gene editing technologies, targeted compounds, manipulation related proteins. This review summarizes different regions intervertebral highlights approaches using modulators ameliorate It discusses complex underscores its potential target.
