Abstract Emergency department overcrowding is a complex problem that persists globally. Data of visits constitute an opportunity to understand it. However, the gap between collected information and real-life clinical processes, lack whole-system perspective, still constitutes relevant limitation. An analytical pipeline was developed analyse one-year production data following patients came from ED (n = 49,938) at Uppsala University Hospital (Uppsala, Sweden) by involving experts in all steps analysis. The key internal issues were high volume generic or non-specific diagnoses non-urgent visits, delayed decision regarding hospital admission caused several imaging assessments beds. Furthermore, external pressure frequent re-visits geriatric, psychiatric, with unspecified dramatically contributed overcrowding. Our work demonstrates through analysis patient flow participation pipeline, it possible identify systemic directions for solutions. A critical factor take whole systems as opened scope boundary effects inflow outflow healthcare system.
