Background and Objectives: In coronary artery bypass grafting (CABG) surgeries, monitoring intracranial pressure (ICP) is crucial due to neurological risks. Although pulsatile flow (PF) during cardiopulmonary (CPB) considered more physiological than non-pulsatile (NPF), its impact on ICP remains unclear. This study aimed compare preoperative postoperative optic nerve sheath diameter (ONSD) measurements between PF NPF techniques evaluate their effect changes. Materials Methods: Sixty patients undergoing elective CABG (aged 45–75 years, ASA II-III-IV) were enrolled divided into two groups depending the technique determined by surgeon: (Group P, n = 30) NP, 30). ONSD performed with ultrasound before surgery (Tpreop) after (Tpostop). Hemodynamic parameters jugular carotid vessel diameters also recorded. Statistical analysis included t-tests, Mann–Whitney U-tests, chi-square tests, Pearson correlation. Results: Both demonstrated significant increases in postoperatively compared values (p < 0.001). However, no statistically difference magnitude of change was observed > 0.05). Group P showed lower ejection fractions higher total inotrope requirements NP 0.01), but these factors did not translate differences dynamics.
