Introduction Shoulder pain is a debilitating musculoskeletal condition with functional, physical, and psychological impacts. Interventions for chronic shoulder should address the biopsychosocial model, Cognitive Functional Therapy (CFT) emerging as promising physiotherapy approach. CFT approaches multidimensional nature of pain, integrating physical cognitive aspects. To date, no study has assessed effectiveness in individuals pain. Therefore, this randomized controlled trial aims to compare effects therapeutic exercises on intensity, disability, self-efficacy, sleep quality, aspects, central processing Methods This will be trial, single-blinded two parallel groups. Seventy-two randomly assigned one groups: or Therapeutic exercise. The interventions last 8 weeks, group receiving therapy once week exercise sessions twice week. primary outcomes intensity while secondary include function, factors, perception improvement/deterioration, processing. outcome measures at baseline, 4 th week, end treatment (8 week), 12 -week follow-up. Conclusion results contribute understanding treating Trial registration number: NCT06542666

Objective: This narrative review aims to analyze physical activity as a central pillar of lifestyle modification in the management chronic musculoskeletal pain by examining its effects on modulation well related domains, including sleep, stress regulation, dietary habits, and smoking behavior. Methods: A structured was conducted. We searched MEDLINE/PubMed, Embase, Cochrane Reviews using terms lifestyle. Randomized controlled trials, observational studies, systematic reviews, reviews reporting concepts interest were included. The results synthesized described narratively. Results: Through release neuromodulatory compounds such endorphins, endocannabinoids, dopamine, serotonin, exercise improves analgesia, promotes emotional resilience, reduces reward response associated with addictive behaviors smoking. Its hypothalamic–pituitary–adrenal axis reduce cortisol levels, while melatonin regulation circadian synchronization deeper sleep stages. In addition, modulates appetite increasing insulin sensitivity altering hormones leptin ghrelin, contributing control energy balance. These mechanisms support comprehensive approach management. Conclusions: Physical is core component lifestyle-based management, not only because analgesic effects, but also positive influence reduction. Although available evidence promising, more randomized trials are needed examine other healthy behaviors, reduction, modification, cessation, consolidate role prevention pain.

Background The McKenzie Method of Mechanical Diagnosis and Therapy (MDT) has long been misunderstood as a biomedical approach. In fact, it is biopsychosocial approach with an up-to-date evidence-based educational curriculum. Recently, become possible to partially measure competence in clinical decision-making based on contemporary pain understanding confidence management using the Pain Understanding Confidence Questionnaire (PUnCQ). primary aim this study was compare following outcomes between physical therapists without credential license MDT (Cred.MDT) PUnCQ well attitudes toward perspective, adherence for low back (LBP), knowledge modern science. secondary explore relevant factors PUnCQ. Methodology Clinical who were managing patients recruited from two associations (the Japanese Society Allied Health Rehabilitation Japan Branch International Institute, had all acquired at least Cred.MDT). measured: (1) PUnCQ-1 partial management; (2) part 2 scores (3) Attitudes Beliefs Scale Physical Therapists (PABS-PT) biopsychosocial/biomedical ratio treatment perspectives; (4) Knowledge (KNAP) science; (5) questionnaire LBP practice guidelines. Two group comparisons conducted multiple regression analysis independent variable aim. Results Data 122 (63 59 participants Cred.MDT, respectively) analyzed. Statistically significantly higher detected Cred.MDT compared those (all p<0.05) above outcomes. demonstrated that statistically significant contributors factor (p = 0.016) acquisition 0.038) (R
