Introduction Shoulder pain is a debilitating musculoskeletal condition with functional, physical, and psychological impacts. Objective: This narrative review aims to analyze physical activity as a central pillar of lifestyle modification in the management chronic musculoskeletal pain by examining its effects on modulation well related domains, including sleep, stress regulation, dietary habits, and smoking behavior. Methods: A structured was conducted. We searched MEDLINE/PubMed, Embase, Cochrane Reviews using terms lifestyle. Randomized controlled trials, observational studies, systematic reviews, reviews reporting concepts interest were included. The results synthesized described narratively. Results: Through release neuromodulatory compounds such endorphins, endocannabinoids, dopamine, serotonin, exercise improves analgesia, promotes emotional resilience, reduces reward response associated with addictive behaviors smoking. Its hypothalamic–pituitary–adrenal axis reduce cortisol levels, while melatonin regulation circadian synchronization deeper sleep stages. In addition, modulates appetite increasing insulin sensitivity altering hormones leptin ghrelin, contributing control energy balance. These mechanisms support comprehensive approach management. Conclusions: Physical is core component lifestyle-based management, not only because analgesic effects, but also positive influence reduction. Although available evidence promising, more randomized trials are needed examine other healthy behaviors, reduction, modification, cessation, consolidate role prevention pain.
