This study examined the community-led urban transformation project for Health New Towns in Senboku Town (Senboku-NT). Senboku-NT is one of largest old new towns (old NTs) South Ward Sakai City, which spans over 1,557 ha with 16 neighborhood units. Regarding historical background, was developed as a quiet residential to provide housing suburban areas 1983. However, population declined approximately 115,000 by 2022. The decline caused an aging population, accounts 37.1% total and demographic change made older people's lives difficult within Senboku-NT. Due regional problems, local communities began explore projects.
