Abstract Background Bear bile powder (BBP), a unique animal-derived medicine with anti-inflammatory and antioxidant effects, is used in Shexiang Tongxin dropping pills (STDP), which applied to treat cardiovascular diseases, including acute myocardial infarction (AMI). The efficacy compatibility mechanisms of action BBP STDP against diseases remain unclear. This study aimed investigate the effects rats AMI. Methods We investigated Non-targeted metabonomics, 16S rRNA analysis, RNA sequencing, network pharmacology were performed explore underlying mechanisms. Results combination CF (STDP without BBP) significantly reduced AMI-induced size, pathological alterations cardiac tissues, serum lactate dehydrogenase creatine kinase levels rats, compared or treatment alone. Gut microbiota metabonomics results revealed that could upregulate relative abundance Lactobacillus downregulate Helicobacter , Bilophila Butyricimonas thereby rebalancing gut dysbiosis induced by Consequently, intestinal metabolite oleoylcholine, glutamylalanine, isokobusone, hemorphin-4 altered. However, alone has weaker effect on these bacteria. Additionally, 62.34% gene reversion rate 55.56% for 30.20% Modulation endothelin 1 growth factor receptor-bound protein 2 was identified as key synergistic mechanism anti-AMI STDP. Conclusion research provides scientific explanation Our findings suggested synergistically attenuates AMI altering expression, microbiota, profiles. Graphical
