Rationale: Sophisticated prognostic scores have been proposed for SARS-CoV-2 but do not always perform consistently. We conducted these meta-analyses to uncover why and investigate the impact of vaccination variants. Methods: searched PubMed database keywords “SARS-CoV-2” with “biomarker” “mortality” baseline tranche (01/12/2020-30/06/2021) either or “Covid19” “vaccination” “variant” from 01/12/2020 31/10/2023. To aggregate data, meta library in R was used, a random effects model fitted obtain pooled AUCs 95% confidence intervals European/North American, Asian, overall datasets. Results: Biomarker effectiveness varies significantly different continents. Admission CRP levels were good marker mortality due wild-type virus Asian countries, area under curve (AUC) 0.83 (95%CI 0.80-0.85), only an average predictor Europe/North America, AUC 0.67 0.63-0.71, P<0.0001). observed same pattern D-dimer IL-6. This variability explains did evenly. Notably, urea troponin had ≥0.78 regardless location, implying that end-organ damage at presentation is key factor. The inflammatory biomarkers (CRP, IL-6) generally declined vaccinated variant cohorts. note significant lag pandemic advent data availability this has no doubt impacted on patient care. Conclusions: efficacies vary considerably by region. It imperative infrastructure collecting clinical should be put place ahead future pandemic.
