Although DNA methylation has been suggested to be a potential predictor of the progression obesity and obesity-related diseases, little is known about its role as predictive marker successful weight loss after bariatric surgery. 20 patients who underwent sleeve gastrectomy were classified according percentage excess (%EWL) 1 year surgery, using 60% cut-off point. Blood was analyzed prior surgery Infinium Methylation EPIC Bead Chip array-based platform. A total number 76,559 differentially methylated positions (DMPs) (p < 0.05) found between <60% EWL >60% groups. Of them, 59,308 DMPs annotated genes. KEGG enrichment analysis showed that pathways involved in signalling MAPK, Wnt, mTor, FoxO AMPK, among others, trajectory. stepwise logistic regression with an absolute Δβ >0.2 higher levels CpG sites cg02405213 (mapping JAK2) (OR: 1.20098, [0.9586, 1.5044]) cg01702330 2.4426, [0.5761, 10.3567]), shown associated probability achieving >60 %EWL gastrectomy, whereas site cg04863892 HOXA5) lower 0.7966, [0.5637, 1.1259]). Abstract Background Type 2 diabetes (T2D) has been linked to changes in DNA methylation levels, which can, turn, alter transcriptional activity. However, most studies for epigenome-wide associations between T2D and comes from cross-sectional design. Few large-scale investigations have explored these longitudinally over multiple time-points. Methods In this longitudinal study, we examined data the Cooperative Health Research Region of Augsburg (KORA) F4 FF4 studies, conducted approximately seven years apart. Leucocyte was assessed using Illumina EPIC 450K arrays. Linear mixed-effects models were employed identify significant sites status, as well with fasting plasma glucose (FPG), hemoglobin A1c (HbA1c), homoeostasis model assessment beta cell function (HOMA-B), insulin resistance (HOMA-IR). Interaction effects status follow-up time also examined. Additionally, CpG associated persistent prediabetes or T2D, progression normal tolerance (NGT) T2D. Finally, identified their corresponding gene expression levels. Results A total 3,501 observations 2,556 participants, measured at least once across two visits, included analyses. We 64 including 15 novel known like those thioredoxin-interacting protein ( TXNIP ) ATP-binding cassette sub-family G member 1 ABCG1 genes. Of these, eight exhibited different rates annual change NGT groups, annotated mannosidase alpha class 2a MAN2A2 carnitine palmitoyl transferase CPT1A ). Longitudinal analysis revealed FPG 128 sites, HbA1c 41 HOMA-IR 57 sites. 104 CpG-transcript pairs whole blood, comprising 40 unique 96 transcripts. Conclusions Our study differentially methylated loci through a approach. report demonstrate that is following differences. These findings provide new insights into molecular mechanisms development. Childhood obesity is a significant global health challenge with rising prevalence over the past 50 years, affecting both immediate and long-term outcomes. The increase in from 0.7% to 5.6% girls 0.9% 7.8% boys highlights urgency of addressing this epidemic. By 2025, it estimated that 206 million children adolescents aged 5-19 years will be living obesity. This review explores complex interplay genomics genetics pediatric obesity, transitioning monogenic polygenic epigenetics, incorporating advancements omics technologies. evolutionary purpose adiposity, systemic evaluation hyperphagia, role various genetic factors are discussed. Technological genotyping offer new insights interventions. integration screening into clinical practice for early identification personalized treatment strategies emphasized.
