Spontaneous intracerebral hemorrhage (ICH) is a major cause of stroke-related morbidity and mortality, with prolonged length stay (PLOS) contributing significantly to patient outcomes healthcare costs. This study aims identify the independent risk factors associated in conservatively treated patients ICH develop predictive model assist clinical decision-making. A retrospective analysis was conducted on 161 spontaneous admitted Second Affiliated Hospital Wannan Medical College between January 2021 December 2022. Demographic, clinical, laboratory data were collected. Factors influencing identified using multivariate logistic regression analysis. nomogram developed predict PLOS, its performance evaluated receiver operating characteristic (ROC) curves, decision curve Patients PLOS group (n = 26, 16.1 %) had higher NIHSS score, systolic blood pressure, rates in-hospital pulmonary infections, white cell counts, as well lower ADL compared those non-PLOS 135, 83.9 %). Multivariate infection (OR 4.548, 95 % CI: 1.529-13.530,P 0.006), score 1.106, 1.017-1.203,P 0.018), =0.974, 0.953-0.996,P 0.022), pressure 1.019, 1.000-1.038,P 0.045) for PLOS. The demonstrated strong discriminatory ability an AUC 0.845, outperforming individual factors. also showed good calibration net benefit, especially when threshold probabilities 10 80 %. incorporating can effectively at after ICH. guide clinicians early intervention strategies resource allocation, ultimately reducing costs improving outcomes.
