Keratoconus (KC) is characterized by irregular astigmatism along with corneal stromal weakness and associated altered immune status. Tissue resident microbiomes are known to influence the status in other organs, but such a nexus has not been described ocular conditions. Therefore, we examined surface microbiome of patients KC correlated it cell tear molecular factor profiles. Sixty-two 21 healthy controls underwent topography analysis eye examination followed collection Schirmer's strip, wash, swabs. Microbiomes were analyzed extracting DNA from swabs 16S rRNA gene V3-V4 amplicon sequencing using QIIME. Fifty-two factors strip extracts 11 cells wash measured multiplex ELISA flow cytometry. Alpha diversity, linear discriminant effect size (LEfSe), relative abundance receiver operating characteristic - area under curve (ROC-AUC) performed. Unsupervised clustering at genus level clinical parameters, soluble factors, was phyla/class, 132 order, 283 family, 718 genera identified our cohort. diversity indices comparable between controls. Dominant phyla across groups Actinobacteria, Proteobacteria, Firmicutes, Bacteroidetes. Alphaproteobacteria increased eyes whereas Firmicutes_Bacilli reduced compared We found significant positive correlation Microbacterium, Cutibacterium, Brevundimonas keratometry thickness. Levels IL-21, IL-9, Fractalkine, VEGF positively Tetrasphaera (P < 0.05). β2-microglobulin CD66bhigh Bacteroides CD45+ Escherichia_Shigella 0.02). discovered unique signature which disease grades secreted cells. on may drive dysregulation provide scope for potential interventions future.
