ABSTRACT Problem There are serious worries about the security and privacy of sensitive patient data as a result growing digitization healthcare systems, particularly when sharing across platforms. Conventional data‐sharing techniques frequently depend on centralized which susceptible to illegal access breaches. Existing encryption techniques, such Blowfish, lack sufficient robustness scalability for modern applications. Additionally, managing keys securely distributed systems remains complex challenge. Aim This work proposes blockchain‐integrated technology with enhanced optimal key generation address these gaps, ensuring secure, scalable, efficient sharing. Methods In proposed model, initiates partitioning into manageable blocks, followed by using an version Blowfish algorithm. The Improved algorithm incorporates bitwise operations conditional transformations strengthen flexibility in handling encrypted data. Also, implements Pelican Optimization Algorithm (IPOA) generation. IPOA optimizes based minimizing correlation between original data, thereby robust protection. A blockchain is used store keys, taking use its decentralized unchangeable structure guard against manipulation unwanted changes. Furthermore, proxy re‐encryption technique employed manage distribute decryption capabilities without exposing information. Results Finally, comparative analyses highlight superiority approach terms security, efficiency, over existing methods. With coefficient 0.203887, exhibits strongest resistance, demonstrating resilience Fault Injection Attack (FIA). Conclusion suggested model's efficacy confirmed, showing that it can improve simplify management, reduce hazards connected conventional procedures.
