Spontaneous rupture of renal masses, known as Wunderlich's syndrome, is a rare but potentially life-threatening condition characterized by acute perirenal hemorrhage. Renal cell carcinoma (RCC) less common cause these ruptures, which may serve the initial manifestation disease. This case report highlights diagnostic and management challenges spontaneous RCC rupture. A 40-year-old female presented with right-sided abdominal pain for 4 h. She exhibited tachycardia (120 bpm). Bedside ultrasonography suggested mass, confirmed contrast-enhanced computed tomography (CECT), revealed right mass large retroperitoneal hematoma. Rapid hemoglobin decline severe raised suspicion Emergency laparotomy nephrectomy was performed, histopathology clear carcinoma. The patient recovered uneventfully. presents therapeutic challenges. Contrast-enhanced (CEST) most reliable imaging modality confirming diagnoses. Differential diagnoses include vascular anomalies, coagulopathies, angiomyolipomas.
