Steroid-induced osteonecrosis of the femoral head (SIONFH) is a universal hip articular disease and very hard to perceive at an early stage. The understanding pathogenesis SIONFH still limited, identification efficient diagnostic biomarkers insufficient. This research aims recognize validate latent exosome-related molecular signature in diagnosis by employing bioinformatics investigate mechanisms SIONFH. GSE123568 GSE74089 datasets were employed conduct differentially expressed genes (DEGs) analysis, dataset was subjected perform weighted co-expression network analysis (WGCNA). (ERGs) retrieved from GeneCards database. We identified (DEERGs) between healthy controls (HC) patients, consensus clustering then implemented group patients. CIBERSORT calculate immune cell infiltration. Gene Set Variation Analysis (GSVA), Ontology (GO), Kyoto Encyclopedia Genes Genomes (KEGG) conducted enriched pathways. In addition, machine-learning algorithms applied refine DEERGs. Ultimately, we verified significance expression hub using performing quantitative reverse transcription polymerase chain reaction (qRT-PCR) analysis. study twenty DEERGs peripheral serum cartilage samples patients HC. Two subtypes related ERGs identified, distinctions pathways infiltration patterns compared. SIONFH's high-risk subpopulation exhibited immune-related high infiltration, such as M0 macrophages, resting mast cells, neutrophils. Three determined LCP1, PNP, UBE2V1, ZFP36 four (ERHGs). Compared HC samples, these ERHGs showed excellent efficiency (overall AUC for range 0.923 0.970 GSE123568) samples. expressions validated finally detected with accordant RT-qPCR. Twenty potential involved through might become candidate therapeutic targets because they have intimate relationship exosomes. These findings shed light on acquaintance contribute prognosis

Ferroptosis is a novel form of regulated cell death characterized by the iron-dependent accumulation lipid peroxides. Recent research has suggested that ferroptosis in osteoblasts contributes to steroid-induced osteonecrosis femoral head (SONFH). However, relationship between and SONFH remains unclear. In this study, vitro experiments show dexamethasone (Dex) treatment reduces expressions key regulators, SLC7A11 GPX4, MC3T3-E1 cells. This reduction leads decrease intracellular glutathione (GSH) levels, accompanied elevated levels total iron, malondialdehyde (MDA), reactive oxygen species (ROS). Importantly, inhibitor ferrostatin-1 (Fer-1) effectively reverses Dex-induced Furthermore, RNA-seq analysis reveals long noncoding RNA (lncRNA) XR_877193.1is significantly upregulated Dex-treated Functional studies demonstrate knockdown lncRNA XR_877193.1 promotes osteogenic differentiation inhibiting cells, whereas its overexpression exacerbates via ferroptosis. Kyoto Encyclopedia Genes Genomes (KEGG) enrichment differentially expressed enriched ferroptosis-related pathways, including PI3K/AKT signaling pathway. Moreover, inhibitors reverse cells inhibited knockdown. Collectively, our findings indicate exerts anti-ferroptosis effects stimulating pathway, suggesting promising therapeutic strategy for attenuating SONFH.
