Antral stricture is a rare cause of gastric outlet obstruction in infants, typically linked to infantile hypertrophic pyloric stenosis (IHPS). We report case 3-month-old male with 1.5-month history progressively worsening, nonbilious vomiting. Although initial imaging suggested IHPS, intraoperative findings revealed sealed antral perforation dense adhesions, leading distal gastrectomy Billroth I reconstruction. A detailed retrospective disclosed that the infant had been given deer velvet powder, suspected have contributed and subsequent formation. This novel association highlights potential risks unregulated supplement use infants underscores need for further investigation.
