The enzyme AAC(6')-Ib-cr belongs to plasmid-mediated quinolone resistance (PMQR), first reported in 2006 and now widely disseminating. Here, we developed three phenotypic methods detect enzyme-producing Enterobacteriaceae (APE), two of which are proposed innovatively this research. These tests based on the following principles: (i) Matrix-assisted laser desorption/ionization time-of-flight (MALDI-TOF MS) can measure mass shift 42 Da resulting from ciprofloxacin acetylation by enzyme. (ii) Co-incubation disks with APE results inactivation drug activity, making it unable inhibit growth indicator organism. We named test method (QIM). (iii) Based principles modified Hodge test, designed (QHT). Through exploration optimal conditions for methods, found that MALDI-TOF MS provides most intuitive after 1 h incubation. interpretability QIM QHT was significantly improved when organism E. coli ATCC25922 replaced a quinolone-slightly-resistant isolate. However, Proteus mirabilis excluded both due its swarming motility. Next, validation study conducted using prospectively collected set 187 clinical strains, demonstrating 100% specificity (MSM: 141/141; QIM, QHT: 135/135) sensitivity 46/46; 33/33) compared genotype. In word, presented simple, efficient, cost-effective detecting APE, suitable microbiology laboratories under various prevention control hospital infections.
