Purpose of review This examines the impact skull base surgery on olfactory function, a critical yet often overlooked aspect postoperative quality life. As surgical techniques continue to evolve, understanding their olfaction is key optimizing patient outcomes. Recent findings The relationship between and function continues be debated in literature. With adoption transnasal approaches, significant concern has been its olfaction. Prior studies have shown evidence dysfunction following surgery, though these are not universal. A particular area discussion involves use pedicled nasoseptal flap, which demonstrated potentially negative short-term impacts. Additional concerns surround flap design (olfactory strip preservation) technique harvest (cold knife versus electrocautery). Evidence suggests that preservation may effectively maintain performance, while cold offer no clear advantage over electrocautery. Summary inconsistencies literature underscore need for standardized, large-scale directly compare better understand essential outcomes improving life postoperatively.
