Introduction: In recent years, significant progress has been made in regenerative medicine, specifically using mesenchymal stem cells (MSCs) due to their and differentiating abilities. An exciting development this area is the utilization of exosomes derived from MSCs, which have shown promise tissue restoration, immune system modulation, cancer treatment.

The intricate and varied clinical presentations of systemic sclerosis (SSc) pose significant challenges for treatment. While several studies have investigated the therapeutic potential mesenchymal stem cell transplantation (MSCT), clarity its long-term outcomes SSc patients is still lacking. Data on MSCT were extracted from medical records inpatients at Nanjing Drum Tower Hospital between January 2013 December 2022. Additionally, Baseline characteristics survival ascertained telephone follow-up. Propensity score matching (PSM) was employed to equalize baseline patient groups, while analysis multivariate Cox regression assessed relationship received all-cause mortality disease-specific rates in patients. Of 333 hospitalized patients, 113 underwent MSCT. log-rank test revealed significantly higher group compared control (10-year rate 89.4% vs. 73.4%, P = 0.002). In PSM cohort, receiving reduced 88.0% 79.9%, 0.032). Multivariate indicated that linked a risk during follow-up period (HR 0.38, 95% CI 0.19–0.75, 0.005). This finding further confirmed matched cohort 0.18–0.82, 0.014). Subgroup analyses treated with correlated various demographics, including younger age diagnosis (≤ 47 years), female, diffuse cutaneous (dcSSc) subtype, concurrent arthritis, pulmonary arterial hypertension (PAH), interstitial lung disease (ILD). enhances SSc, related diagnosis. particularly comorbid conditions, PAH, ILD, digital ulcers, as well those severe associated dcSSc subtype.
