Environmental pollution with heavy metals (HMs) and microplastics (MPs) could enhance the global health challenge antimicrobial resistance (AMR). Herein, we explore complicated mechanics of how HMs, MPs, AMR are interlinked within microbial ecosystems, as well co-selection cross-resistance mechanisms. Unlike antibiotics, HMs have influenced evolution for billions years, promoting mechanisms that predate antibiotic genes (ARGs). At same time, this conundrum is further by pervasive spread MPs in aquatic terrestrial environments, acting substrates bacterial pathogenic biofilms accelerates horizontal gene transfer (HGT) ARGs metal (MRGs). This review highlights such lead (Pb), mercury (Hg), arsenic (As), chromium (Cr), cadmium (Cd), nickel (Ni) persistently selected traits through efflux systems genetic co-regulation. Together, these interactions amplified create exchange hotspots due to biofilm formation. These dynamics modulated organic matter, which serves both a nutrient source mediator HM bioavailability, directly influencing ARG abundance. Soil water including riverine landfill leachate, reservoirs ARG–MRG combinations, notable contributions originating from anthropogenic activities. also emphasizes urgent need integrated environmental public strategies mitigate pollutant-driven AMR. work seeks approach synergistic drivers upstream (causes) levers, foundation future research on sustainable management practices policy (One Health Approach), aimed at curbing determinants can proceed.
