Background The stress hyperglycemia ratio (SHR) has emerged as a potential prognostic indicator for various critical illnesses. However, its role in determining outcomes patients with atrial fibrillation (AF) within the intensive care unit (ICU) remains unclear. This study aimed to elucidate association between SHR and all-cause mortality this clinical setting. Methods We conducted retrospective cohort utilizing data from large, database. Critically ill documented AF were stratified based on quartiles of SHR. primary outcome was 365-day mortality, secondary including 90-day 28-day mortality. COX proportional hazards models adjusted confounders Kaplan-Meier curve analyses used explore relationship Results 2,679 enrolled final study. Among studied, those highest exhibited an increased risk (HR:1.32, 95%CI=1.06-1.65). Notably, subgroup analyses, value particularly pronounced hypertension. Sensitivity confirmed persistence these findings after excluding cohorts malignant tumors, heart failure. Conclusions Our research discerns positive critically AF, highlighting significance acute glycemic dysregulation patient outcomes. Longer follow-up is still needed future AF.

Background Amyloid β1‐40 (Aβ1‐40) contributes to atherosclerosis, being involved in plaque formation and destabilization. The prognostic role of Aβ1‐40 patients with acute myocardial infarction is currently limited non–ST‐segment–elevation (NSTEMI). We examined the value a real‐world cohort (both ST‐segment–elevation [STEMI] NSTEMI) identified predictors for its elevated levels. Methods Results Our population included 1119 consecutive (mean age, 67 years; 72% men; STEMI, 68%). median concentration on admission was 86.9 (interquartile range, 54.5–128.9) pg/mL, there no difference levels between NSTEMI STEMI ( P =0.1). Higher were predicted by older lower left ventricular ejection fraction, glycated hemoglobin >39 mmol/mol glomerular filtration rate <60 mL/min per m 2 . From final multivariable model, nomogram computed determine probability high Aβ1‐40. During follow‐up 57 months, 193 (17.2%) died. Kaplan–Meier analysis revealed higher mortality risk above <0.01), consistent across <0.01) =0.01) subgroups. At Cox including entire cohort, predictive death (hazard ratio, 1.03; =0.01), together high‐sensitivity C‐reactive protein levels, smoking, , worse previous ischemic events. In subcohort, remained significant predictor, along advanced protein. No such association found =0.17), likely due smaller size low event rate. Conclusions an independent predictor improves stratification infarction.

Background/Objectives: Brain insulin resistance is a potential causal factor for dementia in Alzheimer’s disease (AD). Insulin-like growth factor-1 (IGF-1), neurotrophin, plays key role central signaling and neuroprotection. Intracerebrovenitricular (ICV) administration of streptozotocin (STZ) disrupts signal transduction, leading to brain resistance, which may mimic the early pathophysiological changes sporadic AD (sAD). In this study, we investigated whether restoring through ICV injection IGF-1 could ameliorate spatial memory deficits during sAD progression rat model induced by STZ injection. Methods: Male Wistar rats (n = 40) were subjected double injections (0.75 mg/kg/ventricle, days 2 4) (1 μg/single injection, 1 3), placed at Morris water maze (MWM) baseline, 7, 45 90 after injections. Reference (days 1–3 day 4 MWM)) working 5–8 MWM) memory, microglia activation (CD68+ cells), amyloid β (Aβ) deposition (immunohistochemistry) measured. Results: We found that ICVIGF-1 protected demonstrated as (1) reduced latency reach platform, swimming distance trials 3 (p < 0.05) 0.01) on post-injection (2) short-term (up post-injection) enhancement reference manifested reduction 0.05). Furthermore, treatment neuroinflammation CA2 Aβ CA1(p hippocampus. Conclusions: Central attenuates ICVSTZ-induced reducing accumulation
