Introduction Multiple sclerosis (MS) is a chronic neurodegenerative disease that affects over 2.8 million people globally, leading to significant motor and non-motor symptoms. Effective monitoring critical for improving patient outcomes but often hindered by the limitations of infrequent clinical assessments. Digital remote tools leveraging big data AI offer new opportunities track symptoms in real time detect progression. Methods This narrative review explores recent advancements digital MS. We conducted PubMed search collect original studies aimed at evaluating use and/or pwMS. focus on techniques applied from wearable sensors, smartphones, other connected devices, as well AI-based methods analysis data. Results Wearable sensors machine learning algorithms show promise symptoms, such fall risk gait disturbances. Many have demonstrated their reliability not only settings independent execution assessments patients, also passive during everyday life. Cognitive monitoring, although less developed, has seen progress with AI-driven automate scoring neuropsychological tests analyse keystroke dynamics. However, cognitive still underdeveloped, compared Some preliminary evidence suggests application understudied aspects MS (namely sleep circadian autonomic patterns) may provide novel insights. Conclusion Advances exciting possibilities management potential revolutionize care providing continuous, long-term granular both While promising results been demonstrated, larger-scale more robust validation are needed fully integrate these into practice generalise wider population.
