Abstract Background Nonsuicidal self-injury (NSSI) is prevalent in adolescent clinical samples. There evidence that NSSI can be treated effectively but data on individual treatment outcomes limited. The goal of this study was to examine response, remission, exacerbation, and relapse rates over one two years, respectively, among a sample adolescents with NSSI. Furthermore, we aimed identify clinically relevant predictors trajectories. Methods consists n = 203 (12–17 y., 94% female) from specialized outpatient clinic for risk-taking self-harming behavior at least five days the six months before first assessment. Assessments were completed baseline (FU1) (FU2) years later using structured interviews self-report questionnaires. Results At FU1, 75% reported reduction frequency by 50% (treatment response); those, third (25% entire sample) achieved remission (0 NSSI); an exacerbation (⩾50% more NSSI) observed 11% patients. Of those 41% relapsed year later. Predictors non-response or non-remission inpatient depressive symptoms. Adolescents lower had higher risk exacerbation. Due limited size FU2 no prediction model established. Conclusions While most presenting significant improvement, attention should paid rather low full remission. Prediction early detection individuals who deteriorate during after critical.

Non-suicidal self-injury behavior (NSSI) is a serious public health concern that requires immediate attention. Despite the high prevalence of NSSI among Chinese population, there significant gap in research on comprehensive picture this field. Therefore, scoping review was conducted to investigate prevalence, methods, risk factors, and preventive intervention programs related China. The found estimated lifetime youth population alarmingly at 24.7% (N = 1,088,433). Common methods include scratching, hitting, biting. Additionally, synthesized 249 factors based biopsychosocial-ecological framework, highlighting urgent need for intervention. However, only 12 empirical studies focus prevention or were included. These findings underscore necessity more clinical practices larger identify effective interventions ultimately alleviate burden population.FundingThis supported by Humanity Social Science Youth foundation Ministry Education (22YJCZH018), Technology Innovation 2030 (STI2030-Major Projects: 2021ZD0200702), National Natural Foundation China (81825009), Shuimu Tsinghua Scholar. Although child maltreatment (CM) experiences are recognized risk factors for nonsuicidal self-injury (NSSI), the mechanisms underlying this relationship remain unclear. The purpose of study was to examine whether difficulty in emotion regulation (DER) and depressive symptoms mediate between NSSI severity, adjusting demographic variables. The participants were 224 adolescent inpatients recruited from a hospital China (mean age 15.30 years, SD = 1.83; 78.6% females). Study measures included Clinician-Rated Severity Nonsuicidal Self-Injury (CRSNSSI), Childhood Trauma Questionnaire (CTQ-SF), Difficulties Emotion Regulation Scale (DERS), Patient Health Questionnaire-9 (PHQ-9). hypothesized chain mediation model tested using structural equation model.A total 146 (65.18%) adolescents reported engaging during past 12 months, 103 (45.98%) met DSM-5 diagnostic criteria NSSI. Emotional neglect (48.1%) emotional abuse (46.1%) had highest prevalence, followed by physical (43.1%) (24.1%), whereas sexual (12.5%) least prevalent form CM. Separately, both DER significantly mediated association CM NSSI, with being strongest mediator, an indirect effect 49.40% (p 0.014). At same time, we also proved potential chain-mediated pathway depression NSSI.Child seems play role aetiology have mediating Importantly, be mediator stronger compared symptoms.

Abstract Non-suicidal self-injury (NSSI) is a transdiagnostic psychiatric symptom with high prevalence and relevance in child adolescent psychiatry. Therefore, it of great interest to identify biological phenotype associated NSSI. The aim the present study was cross-sectionally investigate patterns markers underlying NSSI psychopathology sample female patients healthy controls. Comprehensive clinical data, saliva blood samples, heart rate variability pain sensitivity, were collected n = 149 40 participants. Using machine-based learning, we tested extent which oxytocin, dehydroepiandrosterone (DHEA), beta-endorphin, free triiodothyronine (fT3), leukocytes, sensitivity able classify participants regarding their outcomes NSSI, depression borderline personality disorder symptomatology. We evaluated predictive performance several models (linear logistic regression, elastic net random forests, gradient boosted trees) using repeated cross-validation. With as an outcome variable, both regression machine learning showed moderate (Area under Receiver Operating Characteristic Curve between 0.67 0.69). Predictors highest power low oxytocin (OR 0.55; p 0.002), 1.15; 0.021), leukocytes 1.67; 0.015). For psychopathological variables, i.e., symptomatology, including variables performed not better than null model. A combination hormonal inflammatory markers, well discriminate without disorder. Based on this dataset, however, complex detect non-linear associations markers. These findings need replication future research will reveal respective biomarkers are useful for longitudinal prediction or treatment response.

Impairments in both stress regulation and emotion recognition have been associated with borderline personality disorder (BPD) non-suicidal self-injury (NSSI). Although it has proposed that deficits particularly emerge during stress, this hypothesis not fully investigated. Adolescents without NSSI performed tasks before after the employment of Trier Social Stress Test (TSST). The psychobiological response was captured psychological self-reports (affect, dissociation), physiological recordings (heart rate, HR, heart rate variability, HRV) endocrinological sampling saliva (cortisol alpha-amylase). Mixed-linear models were applied to analyze stress-induced changes performance respective measures. TSST elicited altered responses adolescents NSSI: A more pronounced decrease positive affect, a increase negative less HRV alpha-amylase throughout induction than NSSI. (dissociation, cortisol HR) differed as function BPD severity on continuum, illustrating greater reactivity but decreased biological responsiveness those severity. had similar effects Recognition sensitivity speed equally increased, absence any differences accuracy. Abstract Heart rate (HR) and vagally mediated heart variability (HRV) are two distinct biomarkers of cardiac autonomic activity. Decreased vagal activity (or decreased HRV) in particular has been linked with impairments the functional flexibility central network (CAN), resulting impaired stress emotion regulatory capacities. HRV is widely used as trait marker psychopathology. Repetitive engagement non-suicidal self-injury (NSSI) adolescence correlates both deficits regulation, well HRV. Existing research has, however, focused on short-term recordings HR under resting phasic conditions. In this study, we examined whether diurnal variation activity, indexed by cosinor parameters derived from 48 h ambulatory ECG recording natural conditions over a weekend, altered female adolescents NSSI disorder compared to controls (HC; N = 30 per study group). Several important confounds, including physical were controlled for. Female show higher rhythm-adjusted 24 mean levels greater respective amplitude HR, lower smaller Peak group reached approximately 1 later HC. Severity exposure early life maltreatment might be amplitudes Diurnal rhythms hold promise objective indicators disordered regulation developmental psychopathology, such should investigated future studies rigorous assessment control potential confounds.
