Background: Awake craniotomy (AC) is recommended for the resection of tumors in eloquent areas. It traditionally performed under monitored anesthesia care (MAC), which relies on hypnotics and opioids. Hypnosis-assisted AC (HAAC) an emerging technique that aims to provide psychological support while reducing need pharmacological sedation analgesia. We aimed compare characteristics outcomes patients who underwent HAAC or MAC. Methods: retrospectively analyzed clinical, anesthetic, surgical, neuropsychological data awake surgical brain used Mann–Whitney U tests, Wilcoxon signed-rank repeated-measures analyses variance identify statistically significant differences at 0.05 level. Results: A total 22 were analyzed, 14 group 8 MAC group. Demographic, radiological, as well postoperative similar. Patients received less remifentanil (p = 0.047) propofol 0.002), but more dexmedetomidine 0.025). None them ketamine a rescue analgesic. Although experienced higher levels perioperative pain < 0.05), they reported decreasing stress 0.04) greater satisfaction 0.02). Conclusion: safe alternative it reduces increases overall satisfaction. Further research necessary assess whether hypnosis clinically beneficial.

Abstract In recent years, it has been increasingly recognized that tumor growth relies not only on support from the surrounding microenvironment but also tumors capacity to adapt – and actively manipulate its niche. While targeting angiogenesis modulating local immune environment have explored as therapeutic approaches, these strategies yet yield effective treatments for brain remain under refinement. More recently, nervous system itself a critical environmental cancer, with extensive neuro-tumoral interactions observed both intracranially in extracranial sites containing neural components. brain, between glioma cells well metastatic lesions components clinical implications diagnostics, risk assessments, neurological sequelae, development of innovative therapeutics. Here, we review dynamics, emphasizing aspects relevant neurosurgical practice.

Abstract Background Inferior Fronto-Occipital Fascicle (IFOF) is a multitasking connection bundle essential for communication and high level mentalization. The aim of the present study was to quantitatively assess its radiological-anatomical-morphometric modifications according different brain tumor histotype. Methods A retrospective multicentric Italian conducted. IFOF reconstructions were calculated both hemispheres each patient diagnosed with Glioblastoma (GBM), Low Grade Glioma (LGG), Brain Metastasis Meningioma using Elements Fibertracking software (Brainlab AG). 3D object fascicle evaluated volume, average fractional anisotropy (FA) length. cerebral healthy hemisphere compared pathological contralateral in Results 1294 patients evaluated. 156 met inclusion criteria. We found significant difference between mean length volume ( p -value < 0.001). Considering GBM subgroup, Student’s t-test confirmed results. In LGG there 2 length, FA (respectively 0.011; 0.021, affected by metastasis (18) showed 0.003 0.02 respectively). No differences meningiomas. Conclusions careful preoperative neuroradiological evaluation brain-tumor interface indispensable plan tailored surgical strategy perform safe effective technique. It depends on histology pattern growth. have mixed component, solid enhancing nodule which accounts displacement peritumoral area an infiltrative/destructive effect fascicle. determine prevalent infiltrative pattern. Metastases dislocation due oedema. Meningiomas do not impact WM anatomy.

DLAT has been recognized as a cuproptosis-related gene that is crucial for cuproptosis in earlier research. The study to look at how affects individuals with low-grade glioma's prognosis and immune infiltration. Genotype-Tissue Expression (GTEx) database the TCGA were used this work download RNAseq data TPM format. was found be overexpressed LGG by comparing expression levels between normal brain tissue, of verified immunohistochemistry semi-quantitative analysis. Then, functional enrichment analysis revealed biological pathways possible signal transduction involved primarily focused on extracellular matrix organization, transmembrane transporter complex, ion channel activity, neuroactive ligand-receptor interaction, complement coagulation cascades, activity. level cell infiltration plasmacytoid dendritic cells CD8 T subsequently evaluated using single-sample set analysis, which showed high inversely connected link methylation mRNA transcription then further investigated via MethSurv database, results DLAT's hypomethylation status linked poor outcome. Finally, evaluating prognostic value Cox regression Kaplan-Meier technique, column line graph created forecast overall survival (OS) rate 1, 3, 5 years after identification. aforementioned demonstrated significantly decreased OS DSS, overexpression WHO grade, IDH status, primary therapy outcome, (OS), disease-specific (DSS), progression-free interval (PFI) events. discovered separate predictive sign end. might thus represent brand-new biomarker.
