The cerebellum has long been associated with motor control. However, its role in cognitive functions attracted increasing attention recently. uniformity of cerebellar internal structure seems at odds involvement such diverse functions. Nonetheless, diseases as ataxia, there is a comorbidity and impairments, raising essential questions about how to what extent the participates This review begins by tracing historical development research, suggesting that connections between cerebral cortex, basal ganglia, other subcortical nuclei form basis for cerebellum’s regulating We then delve into language, reward-based learning, working memory, spatial cognition. Additionally, we summarize changes observed Alzheimer’s disease (AD), Parkinson’s (PD), ataxias their impact on By discussing mechanism cognition physiology pathology from aspects function, aim shed light promising new therapeutic targets related impairment.

Abstract New combinations or alternative therapies for major depressive disorder (MDD) are necessary. Intradermal acupuncture (IA) shows promise but requires further investigation regarding its efficacy, safety, and mechanisms. Conducted across 3 centers from November 2022 to January 2024, our randomized controlled trial included 120 participants with moderate severe MDD, divided into the selective serotonin reuptake inhibitors (SSRIs), SSRIs plus sham IA (SSRIs + SIA), active AIA) groups. Acupuncture groups received 10 sessions over 6 weeks at Shenmen (HT7), Neiguan (PC6), Sanyinjiao (SP6) Taichong (LR3) bilaterally, followed by a 4-week follow-up. The primary outcome was changes in Hamilton Depression Rating Scale-17 (HAMD-17) scores week 6. Furthermore, healthy controls (HCs) MDD patients underwent magnetic resonance imaging (MRI) scans functional connectivity (FC) analysis. After of treatment, AIA group showed greater reduction HAMD-17 score than SIA (MD, −4.9 [CI, −7.6 −2.2], P < 0.001) −5.1 −7.8 −2.3], 0.001). No serious adverse events occurred. resulted lower incidences palpitations (vs.SSRIs SIA: OR, 0.1% 0.0–1.0%]; vs. SSRIs: 0.0–0.7%]; 0.05), somnolence 0.0–0.9%]; vs.SSRIs: nausea 0.05). abnormal FCs, enhanced FCs between striatum frontal_inf_tri, cerebellum MRI study. Overall, as adjunctive therapy provides clinical efficacy safety it may exert antidepressant effects modulating striatal FCs.
