Myocardial infarction (MI) is associated with emotional distress, depression, and anxiety. Defense mechanisms mentalized affectivity play key roles in affective regulation; however, their individuals MI remain underexplored. This study examined the relationship between depressive anxiety symptoms, defensive functioning, patients. Sixty-seven patients 80 healthy controls completed DSM-5 Self-Rated Level 2 Cross-Cutting Symptom Measures—Depression Anxiety—Adult, Mechanisms Rating Scales-Self-Report-30, Brief-Mentalized Affectivity Scale. Multivariate analyses were used to compare groups, while correlations mediation models evaluate associations variables within group. Compared controls, exhibited more severe maladaptive defenses, lower levels of affectivity. Within group, severity symptoms was immature functioning poorer abilities regulate affects. Notably, worse mediated by capacity process emotions. The mediating role this specific dimension emphasizes potential affect regulation as a target for tailored psychosocial interventions aimed at improving clinical outcomes.
