The association between social determinants of health and all-cause, cardiovascular mortality in US rheumatoid arthritis: a cohort study of NHANES

Rheumatoid arthritis (RA) imposes a significant burden on healthcare systems. This study investigates the impact of social determinants health (SDoH) prognosis RA patients, aiming to inform interventions that improve outcomes. The National Health and Nutrition Examination Survey (NHANES) 2005-2018 included 2,024 participants. SDoH was assessed based five domains outlined in Healthy People 2030 framework, with cumulative number adverse calculated. Cox regression analysis, subgroup Kaplan-Meier survival analysis were employed evaluate associations between all-cause mortality as well cardiovascular among adjusting for potential confounding factors. Over median follow-up 79 months, 449 deaths (19%) occurred, including 120 deaths. Each 1-point increase score linked 24% higher risk (HR: 1.24; 95% CI: 1.16-1.32; p < 0.001) 25% 1.25; 1.13-1.39; 0.001). Exposure or more associated 3.94; 2.20-7.06; 3.98; 1.22-12.97; = 0.020). Significant interactions found age, income, education level. Adverse SDoH, particularly when accumulated, are increased risks emphasizing need targeted mitigate these effects.
