Background The influence of digital financial inclusion on medical insurance systems has drawn significant attention. However, its impact China’s multilevel system (MMIS) remains uncertain. MMIS is crucial for societal wellbeing. It safeguards citizens from hardships during treatment, especially as the population ages and healthcare costs rise. By covering expenses, ensures equal access to essential services, protecting people’s health rights preventing impoverishment caused by thus promoting social stability equity. This study used panel data 31 provinces in China 2016 2020 explore development MMIS. Methods An matrix was constructed using entropy weight method Technique Order Preference Similarity an Ideal Solution (TOPSIS). Panel regression models were employed analyze effects different dimensions Results overall index, particularly “coverage” dimension, had a positive ( p < 0.05). “depth” “digitization” statistically insignificant > 0.1). Robustness checks lagged instrumental variable approaches double machine learning (DML) methods confirmed these findings. Heterogeneity analysis causal forest model revealed that more pronounced effect central western regions. Its eastern regions relatively weaker, highlighting regional disparities. Conclusion Digital infrastructure must be strengthened enhance coverage service capacity; depth quality services should improved optimize finance Deeper integration between prompted through technological innovations improve both equity sustainability. provides valuable insights policymakers address disparities further developing
