The COVID-19 pandemic adversely impacted migrants in Malaysia, raising concerns about the effectiveness of public health measures. This study aims to investigate challenges faced by migrant populations complying with measures during pandemic. We conducted 29 in-depth interviews stakeholders between April 2022 and February 2023. Thematic analysis was conducted, results were organised major measures: (1) movement restrictions, (2) non-pharmaceutical interventions, (3) screening testing (4) quarantine, isolation hospitalisations. Migrants encountered difficulties control orders due livelihood crises a lack understanding regulations. Financial constraints hindered migrants' ability purchase quality face masks, they lacked comprehension importance interventions for disease prevention. In absence government intervention, non-governmental organisations international played an important role providing essential food aid, information, masks hygiene products, other services migrants. Despite encouragement seek treatment, deterred fear immigration enforcement unaffordable fees. Overcrowded living conditions made physical distancing, quarantine challenging. Many avoided government-designated centres financial arrest. Delayed medical treatment may have resulted high mortality among highlighted significant disparities experienced including double crises, limited access resources, healthcare social protection. Urgent reforms are needed ensure migrant-inclusive policies, enhance outbreak preparedness prevent unnecessary suffering deaths both non-pandemic periods.
