Osteoarthritis (OA) is a debilitating degenerative disease of the joints and one most prevalent joint disorders affecting millions individuals worldwide. This highlighted by significant morbidity owing to encumbering pain functional impairment. OA ensues following disruption normal homeostasis in resulting from aging, metabolic changes, or as consequence injury (referred post-traumatic OA). These processes are largely driven low-grade inflammation that gradually compromises anabolic protective activities resident cells including chondrocytes, synovial fibroblasts (SFs) immune cells. Ample research suggests process cartilage deterioration endpoint complex pathologic culminating with synovitis, subchondral bone sclerosis, osteophyte formation, aberrant remodeling, ultimately articular degradation. There remains great need for identifying early markers "window opportunity" enable timely interventions OA. However, this effort hampered nature its comorbidities. Joint holistic approaches using recent unbiased multi-omic tools currently at forefront promising better understanding development. Currently, there no meaningful disease-modifying drugs treat OA, surgical procedures ultimate effective intervention end stage patients. The disability, pain, costs associated management position among costliest onerous our society. mini review will highlight advances last two decades major obstacles limiting progress particular emphasis on inflammatory
