Leptomeningeal metastasis (LM) is a fatal neurological complication of cancer. Proton craniospinal irradiation (pCSI) has emerged as promising life-prolonging intervention for LM patients, but the response to this treatment varies. Here, we aimed characterize molecular basis pCSI resistance and response. Proteomic analysis CSF collected from patients at baseline (before pCSI), multiple time points post-treatment, identified CXC-motif chemokine, CXCL1, associated with growth. Higher CXCL1 levels in prior correlated worse treatment. To define role LM, established syngeneic mouse models LM-CSI. We found that both metastatic cancer host cells generate CXCL1. Genetic interruption Cxcl1 expression cancer, not cells, impaired cell growth within leptomeninges. Moreover, subset expressed Cxcr2, primary receptor Cxcl1, population was enriched over Transcriptomic profiling rare revealed an enrichment pathways implicated cycle progression. Finally, Cxcl1-Cxcr2 signaling intrathecally-delivered Cxcr2 antagonist hampered sensitized CSI. Our results demonstrate axis mediates growth, identifies potential actionable improve halt CXCL1-CXCR2 therapeutic target leptomeningeal progression enhance irradiation.

Leptomeningeal carcinomatosis (LC) is a severe complication in the advanced stage of lung adenocarcinoma, with an extremely poor prognosis. Currently, diagnosis LC poses challenges. Serum exosomal miRNAs (microRNAs) have been demonstrated to possess potential as viable biomarkers. However, their value remains unclear. In this study, serum samples were collected from adenocarcinoma patients LC. The control groups consisted early-stage and advanced-stage without exosomes isolated for high - throughput RNA sequencing screen differential miRNAs, results validated 123 samples. Subsequently, receiver operating characteristic (ROC) curve was used evaluate diagnostic ability miRNA revealed seven differentially enriched (miRNA-1296-5p, miR-503-5p, miR-499a-5p, miR-374a-5p, miR-3173-5p, miR-370-3p miR-885-3p). ddPCR confirmed that expression levels miRNA-1296-5p, miR-499a-5p miR-374a-5p significantly elevated LC, while decreased (P < 0.05). ROC analysis showed AUC combination CEA 0.803 0.0001), displaying higher power This study suggests specific which has potential. And can further enhance

Leptomeningeal metastasis of gastric adenocarcinoma (LM-GC) is a rare and severe complication with poor prognosis, its prognosis significantly poorer than liver, lung, peritoneal metastases. Studies on LM-GC have been limited to clinical case reports. Despite advances in systemic therapies, there lack standardized treatment protocols for due rarity the challenges it presents.
