Introduction Adrenal tumors (ATs) encompass a wide differential diagnosis, necessitating multi-step process for accurate identification. Liquid biopsy emerges as promising non-invasive technique distinguishing between malignant and benign, well hyperfunctioning non-functioning cases. Recent studies have highlighted the potential of microRNAs circulating biomarkers; however, their clinical utility remains underexplored. This study aims to validate diagnostic performance selected microRNAs, (miR-483-5p, miR-210, miR-335 miR-22-3p), identified through microRNA profiling studies, markers malignancy or cortisol hypersecretion in cohort patients with ATs. Methods We collected serum samples from 75 ATs, including 50 cases adrenocortical adenomas (ACA) 25 carcinomas (ACC), along 15 controls. In ACC subgroup, 16 were obtained preoperatively upon detection recurrence (active group), while remaining disease-free long-term follow-up. Among 56 ATs evaluated (50 ACAs 6 ACC), 26 had tumors, 22 exhibited mild autonomous secretion, 8 Cushing syndrome. Quantitative real-time polymerase chain reaction was employed analyze expression. Glycosylation is a crucial post-translational modification that regulates cellular functions and the surrounding microenvironment. The abnormal expression of glycosyltransferases, essential enzymes in this process, has been linked to tumor development.
