The field of vascularized composite allografts (VCAs) has undergone significant advancement in recent decades, and VCAs are increasingly common accepted the clinical setting, bringing hope functional recovery to patients with debilitating injuries. Long-term systemic immunosuppression is needed for vascularized composite allotransplantation (VCA). The high rate of acute rejection episodes in the first posttransplant year, development chronic rejection, and adverse effects that come along with this treatment, currently prevent a wider clinical application VCA. Opportunistic infections metabolic disturbances are among most observed side VCA recipients. To overcome these challenges, local using biomaterial-based drug delivery systems (DDS) have been developed. aim to provide concentrations immunosuppressive drugs while reducing their load. This review provides summary recently investigated DDS different mechanisms action such as on-demand, ultrasound-sensitive, or continuous delivery. In preclinical models, ranging from rodent porcine nonhuman primate approach has shown reduce tacrolimus (TAC) load effects, prolonging graft survival. Localized represents an encouraging enhance survival toxic patients. Preclinical models TAC-releasing demonstrated low burden. However, events translational animal reality, use additional low-dose TAC treatment may be envisaged. Patients benefit through efficient negligible resulting better compliance quality life.
