Long-term systemic immunosuppression is needed for vascularized composite allotransplantation (VCA). The high rate of acute rejection episodes in the first posttransplant year, development chronic rejection, and adverse effects that come along with this treatment, currently prevent a wider clinical application VCA. Opportunistic infections metabolic disturbances are among most observed side VCA recipients. To overcome these challenges, local using biomaterial-based drug delivery systems (DDS) have been developed. aim to provide concentrations immunosuppressive drugs while reducing their load. This review provides summary recently investigated DDS different mechanisms action such as on-demand, ultrasound-sensitive, or continuous delivery. In preclinical models, ranging from rodent porcine nonhuman primate approach has shown reduce tacrolimus (TAC) load effects, prolonging graft survival.
