Severe asthma is associated with an increased risk for exacerbations, reduced lung function, fixed airflow obstruction, and substantial morbidity mortality. The concept of remission in severe as a new treatment goal has recently gained attention due to the growing use monoclonal antibody therapies, which target specific pathologic pathways inflammation. This review evaluates current definitions unveils some barriers achieving this state population. Although there no unified definition, clinical should be based on sustained period symptom control, elimination oral corticosteroid exposure stabilization pulmonary function. conjugation these criteria seems realistic minority asthmatic patients. Some unmet needs may affect achievement remission. Late intervention targeted therapies population increase development irreversible structural airway changes. Early-life respiratory infections and asthma are major health burdens during childhood. Markers predicting an increased risk for early-life diseases sparse. Here, we identified the predictive value of ultrasound-monitored fetal lung growth asthma. Fetal size was serially assessed at standardized time points by transabdominal ultrasound in pregnant women participating a pregnancy cohort. Correlations between infancy or early-onset five years were examined. Machine-learning models relying on extreme gradient boosting regressor classifier algorithms developed to predict infection based growth. For model development validation, study participants randomly divided into training testing group, respectively, employed algorithm. Enhanced throughout predicted lower risk. Male sex associated with higher infancy. could also age. We designed three machine-learning number as well The models' R2 values 0.92, 0.90 0.93, underscoring high accuracy agreement actual values. Influential variables included known factors novel predictors, such Sonographic monitoring allows

In the field of severe asthma, concept disease control has recently been integrated by one clinical remission. With this new concept, we move on to analyze efficacy therapy multiple parameters simultaneously, starting with mandatory discontinuation systemic glucocorticoids, which is added effect exacerbations, respiratory function, and symptoms control. The Italian asthma registry SANI (Severe Asthma Network Italy) drafted criteria for definition remission, allowing patients be classified into two groups, partial complete greater dynamism definition, provided SANI, allows us hypothesize its practical use, concerning management patients, from level aim facilitate decision replacement, continuation or modulation patients' therapy.

The development and approval of DMAADs ("disease-modifying anti-asthmatic drugs"), in particular inhaled steroids (alone or combination with long-acting bronchodilators), biologics modern allergen immunotherapies, has fundamentally changed the asthma therapy concept from symptom control to prevention. This is linked new treatment goal remission: long-term absence symptoms (good control), exacerbations, stable lung function, without use systemic for therapy. Three types remission are distinguished: spontaneous (e.g., childhood asthma), "off treatment" after successful immunotherapy), "on during biologic therapy). A treat-to-target approach used, as rheumatoid arthritis chronic inflammatory bowel disease: achieve remission, through individually tailored highly effective drugs minimal side effects. However, this requires precise phenotyping patient, including detailed history taking, pulmonary function diagnostics, allergological measurement type 2 biomarkers.

Asthma is associated with atherosclerosis and abdominal aortic aneurysm (AAA). However, the underlying pathomechanisms remain elusive. Blood eosinophil count (BEC) implicated in both eosinophilic asthma arterial wall inflammation. To explore possible association of BEC artery changes. 112 outpatients were prospectively enrolled this exploratory study. Abdominal diameter was measured using ultrasonography imaging, while vascular speckle tracking utilized to evaluate strains. Patients stratified into two groups, n=66 patients a ≥300 n/µL n=46 <300 n/µL. Both groups exhibited no significant disparities cardiovascular risk factors; however, high group more frequently male. The wider (1.46 ± 0.25 cm vs 1.67 0.63 cm, p=0.018). Three diagnosed an AAA, all had lower strain values, indicative higher stiffness, including radial (2.65 1.38% 4.46 2.59%; p<0.001). positive correlation (R²=0.131, b=0.000, p<0.001), negative values b=-0.002, p=0.001) sex-adjusted linear regression. In asthma, blood correlated heightened stiffness aorta. Hence, they may be at elevated developing AAA.
