Treatment-resistant late-life depression (TRLLD) affects nearly half of older adults with major depression. This systematic review evaluates published evidence effectiveness both pharmacological and non-pharmacological treatments for TRLLD. A search MEDLINE, EMBASE, CINAHL, PsycINFO, the Cochrane Library, online trial registries up to March 2024 was conducted identify randomized controlled trials (RCTs) evaluating interventions Seven studies assessed (antidepressants, antipsychotics, mood stabilizers, or ketamine) another seven examined approaches (psychotherapy, electroconvulsive therapy, repetitive transcranial magnetic stimulation (rTMS), computerized cognitive remediation). Aripiprazole (2 studies), venlafaxine (1 study), ketamine lithium study) were associated a reduction in depressive symptoms post-treatment compared comparator treatment group. rTMS sequential bilateral theta burst remediation also showed significant improvements Quality varied from very low medium among included studies. Most reported data on small sample sizes. We identified number RCTs augmentation appears be an effective based two studies, acceptable side effect profile. Other may effective, but is low-quality evidence. Future large-scale are urgently needed draw firm conclusions.

Depression, projected to be the predominant contributor global disease burden, is a complex condition with diverse symptoms including mood disturbances and cognitive impairments. Traditional treatments such as medication psychotherapy often fall short, prompting pursuit of alternative interventions. Recent research has highlighted significant role gut microbiota in mental health, influencing emotional neural regulation. Fecal transplantation (FMT), infusion fecal matter from healthy donor into patient, emerges promising strategy ameliorate depressive by restoring microbial balance. The microbial-gut-brain (MGB) axis represents critical pathway through which potentially rectify dysbiosis modulate neuropsychiatric outcomes. Preclinical studies reveal that FMT can enhance neurochemicals reduce inflammatory markers, thereby alleviating behaviors. Moreover, shown promise clinical settings, improving gastrointestinal overall quality life patients depression. review highlights gut-brain depression need for further validate long-term safety efficacy FMT, identify specific therapeutic strains, develop targeted modulation strategies. Advancing our understanding could revolutionize treatment, shifting paradigm toward microbiome-targeting therapies.

ABSTRACT Background In recent years, cognitive impairment has emerged as a pivotal symptom in elderly patients with depression, exerting substantial impact on the course and prognosis of diseases. Moreover, it persists even following remission from depression during rehabilitation period. However, there remains an incomplete understanding relevant influencing factors for depressed patients, which seriously impedes development risk prediction models subsequent research precision intervention programs. Objective The purpose this study is to examine current state negative assessment tools late‐life (LLD), thereby providing theoretical framework construction targeted Methods search strategy employed followed evidence‐based approach, utilizing systematic scoping review thoroughly explore six English four Chinese databases up until November 2023. Two researchers independently conducted article screening thematic analysis categorize results into themes. Results Following two rounds rigorous by team, data were meticulously extracted succinctly summarized five distinct themes encompassing socio‐demographic, physiological, psychological, genetic, other related factors. addition, comprehensive compilation 19 diverse was undertaken. Ultimately, total 22 articles met eligibility criteria inclusion study. These comprised longitudinal studies, nine pathological controlled cross‐sectional cohort one randomized Conclusion Cognitive dysfunction important LLD, affects survival patients. Materials Methods: retrospective conducted at Lucerne Cantonal Hospital Wolhusen, Switzerland, spanning from 2015 2020 including 1159 individuals aged 65 years older. presence assessed using Geriatric Scale (GDS) Short Form, while abilities were evaluated Functional Independence Measure (FIM) Tinetti test. Data analysis performed TIBCO Statistica 13.3, with statistical significance set p < 0.05. Results: Of participants, 22.9% (N = 266) exhibited symptoms, no notable differences between genders. Although all showed improvements, duration prolonged by two days (p 0.012, d 0.34) those symptoms. Alarmingly, 76% participants classified as risk falling based score. However, correlation found GDS scores admission 0.835, r 0.211) or discharge 0.336, 0.184). results non-parametric Wilcoxon matched-pairs test provide compelling evidence changes FIM when comparing across categories. Abstract Major depressive disorder is a common mental disorder, and significant number of patients exhibit poor response to conventional antidepressant treatments, known as treatment-resistant depression (TRD). However, the definition TRD globally remains unclear, impeding clinical research, treatment development, outcome implementation, policy-making. A wealth research confirms that rTMS demonstrates promising efficacy in TRD. This paper elucidates TRD, summarizes potential targets for treating comprehensively elaborates on unique mechanisms, efficacy, side effects outlines considerations special populations receiving well other modalities Through these studies, we aim provide more scientifically grounded recommendations undergoing
