The 2-component molecular-level model dividing body mass into fat and fat-free (FFM) is a cornerstone of contemporary composition research across multiple disciplines. Confusion prevails, however, as the term lean (LBM) frequently used interchangeably with FFM in scientific discourse. Are LBM same or different components? Captain Albert R. Behnke originated concept 1942 he argued that his "physiological" component included "essential" structural lipids whereas chemical entity "free" fat. Classical experimental animal human studies conducted during Behnke's era laid foundation for widely density total water models. Refined models, organization functional groupings, lipid extraction methods all have advanced since era. Our review provides an in-depth analysis these developments aim clarifying distinctions between FFM. retrospective reveals FFM, derived experimentally difference weight extracted neutral nonpolar (mainly triglycerides), includes polar (that is, fat). Accordingly, originally proposed by has thus answering longstanding ambiguity literature. Bringing science modern mandates use chemically correct elimination duplicative today value primarily historical context. Avoiding additionally limits confusion surrounding similar terms such mass, soft tissue muscle mass.

Abstract Objective When treated with a macronutrient‐balanced hypocaloric diet, do male individuals who have overweight and obesity lose relatively more dual‐energy x‐ray absorptiometry (DXA)‐measured lean soft tissue (LST) mass than female individuals? Are there changes in bone mineral content (BMC), if so, how they impact relative reductions LST compared to fat‐free (FFM; plus BMC)? decrements fat, LST, FFM predictable from the magnitude of weight loss or baseline body composition? Methods To answer these questions, DXA studies were conducted before after 9‐ 12‐week calorie‐restriction period 43 97 lost mean (SD) 10.8% (2.2%) 10.7% (1.6%) their weight, respectively. Results The proportion as was significantly ( p < 0.001) larger (mean [SD], 0.33 [0.11] kg) (0.25 kg); BMC paradoxically increased, thereby leading smaller reduction (−3.87 [1.73] kg vs. −3.92 [1.74] kg; (−2.22 [1.18] −2.24 0.001), three different analyses showed that composition tracked predicted priori change composition. Conclusions These observations provide insights into future guidance for analyzing DXA‐measured associated newer pharmacotherapies loss.
