ABSTRACT Background Meningitis due to Cryptococcus neoformans continues cause substantial morbidity and mortality in victims of this infection. Worldwide, immunosuppression uncontrolled HIV disease is the main risk factor. In United States, however, a percentage cryptococcal meningitis cases occur people with non-HIV immune compromising conditions such as solid organ transplantation. Neurological damage can be caused by direct fungal-induced injury, consequence elevated intracranial pressure, immune-mediated injury. An understanding these pathophysiological processes an appreciation for roles specific treatments address them are necessary optimal care patients meningitis. Case Summary report, we present case liver transplant recipient whose manifestations were (i) fungal invasion, (ii) (iii) over-exuberant response that blossomed soon after reduction clearance cerebrospinal fluid culture.
