In this report we describe the case of a healthy, young, athletic woman who developed acute lymphoblastic leukaemia (ALL)/lymphoblastic lymphoma (LBL) after receiving second dose Pfizer/BioNTech modified mRNA (modRNA) COVID-19 genetic vaccine (marketed as Comirnaty®). The first did not appear to illicit any noticeable side effects, but within 24 hours patient suffered widespread and intensifying bone pain, fever, vomiting, general malaise. Due persistence symptoms, underwent series tests examinations including full laboratory workup, consult with clinical immunologist rheumatologist, Positron Emission Tomography (PET) imaging, well an osteomedullary biopsy. Objectives new-onset relapsing-remitting multiple sclerosis (RRMS) has rarely been communicated as a side effect of SARS-CoV-2 vaccination (SC2V). Here we report patient with RRMS shortly after the second BNT162b2 vaccine dose, which was diagnosed only relapse third dose. Case is 31 year-old male who developed eye problems 48 days dose BNT162b2. The symptoms were attributed to strain and gradually disappeared without treatment. Further workup negative. Three months urinary urgency developed. Eleven he sensory visual disturbances that noticeable post-exercise progressed worsened alcohol heat. MRI showed typical lesions met Barkhof/Tintore criteria for RRMS. Cerebrospinal fluid (CSF) studies revealed elevated kappa free light chains positive oligoclonal bands. Background/Objectives: While post-acute COVID-19 syndrome is well known and extensively studied, the COVID vaccination (PACVS) a more recent nosological entity that poorly defined at immunopathological level, although it shares many symptoms with sequelae of viral infections. Methods: This single-center retrospective study reports case series 17 subjects vaccinated mRNA or adenoviral vector vaccines who were healthy before had never been infected SARS-CoV-2 but presented similar to PACVS for median time 20 months (min 4, max 32). The medical records all patients referred our outpatient clinic over one-year period retrospectively analyzed.
