Limited evidence exists on the gender-specific impact of changes in loneliness, social isolation, and support dementia risk. We examined these their relationships with cognitive decline. Data from over 12,000 community-dwelling Australians aged 70+ years without significant impairment at enrolment were analysed. Loneliness, self-reported baseline ∼2 later (social isolation support) or ∼3 (loneliness), classified as never, transient, incident, persistent. Dementia diagnosis followed DSM-IV criteria, adjudicated by an expert panel. Gender-disaggregated Cox proportional hazards regressions conducted, adjusting for age other risk factors. At baseline, participants 70-95 (mean: 75.2 ± 4.3), 54% being women. Overall, 81.1% men 71.7% women reported never feeling lonely while persistent loneliness experienced 4.9%, 8.4%, 5.5% 8.5%, 11.6%, 8.3% women, respectively. Over a median 8-year follow-up, incident (HR: 1.52, 95% CI: 1.08-2.13) 2.14, 1.55-2.97) associated greater risk, compared to those who lonely. No increased was observed transient loneliness. Despite remarkably low prevalence poor this initially healthy cohort, both decline (secondary outcome) but not Persistent people 70+, especially higher

Abstract Aims A systematic review and meta-analysis was conducted to investigate the prevalence antecedents/outcomes of loneliness social isolation among individuals with severe mental disorders (SMD), such as schizophrenia, schizoaffective disorder, bipolar disorder or major depressive disorder. Methods Five well-known electronic databases (PubMed, PsycINFO, CINAHL, Web Science Scopus) were searched (plus a hand search). Observational studies that report and, if available, antecedents consequences loneliness/isolation SMD included. Key characteristics extracted, performed. Our preregistered on PROSPERO (ID: CRD42024559043). The PRISMA guidelines followed. Joanna Briggs Institute (JBI) standardized critical appraisal tool developed for applied assess quality included studies. Results initial search yielded 4506 records, after duplicate removal screening, total 10 finally used data from Europe, Asia, North America, Oceania. Two employed longitudinal design, while all other had cross-sectional design. Most between 100 500 SMD. All involved both male female participants, women typically comprising about 40% sample. average age participants often ranged approximately 30 40 years. estimated 59.1% (95% CI: 39.6% 78.6%, I 2 = 99.3, P < .001) any diagnosis Furthermore, objective 63.0% 58.6% 67.4%) schizophrenia spectrum moderate good. Subjective well-being symptoms in particular found contribute Conclusions present identified high levels those These findings stress importance monitoring addressing needs this vulnerable group, which may have positive effect life Future research neglected regions (e.g. South America Africa) is recommended. Different diagnoses within should be distinguished future additional are required explore
