Abstract Semi‐synthetic cannabinoids (SSCs) including hexahydrocannabinol (HHC) are emerging on the drug market and sold openly as purportedly legal replacements for cannabis Δ 9 ‐THC. By beginning of 2024, 24 European countries had identified HHC, often in edibles (foods/candy), vapes low‐THC flowers resins. The SSC is developing rapidly, with HHC acetate (HHC‐O), hexahydrocannabiphorol (HHC‐P) others recently identified. These developments may mark first major change ‘legal’ to since ‘Spice’ containing synthetic cannabinoids, such JWH‐018, emerged 2008. Currently, there some data available pharmacology SSCs, which crucial understanding their effects, evaluating health risks informing public responses. This study focused characterizing vitro activation human CB 1 receptor by ( R ) ‐ S epimers HHC‐P HHC‐O. Using recombinant CHO‐K1 cells expressing receptor, potency (EC 50 efficacy were determined. It was established that (9 )‐HHC )‐HHC‐P activated partial agonists five two times lower compared respectively, while exhibited even potency. epimer HHC‐O activate lesser extent )‐epimer showed no activation. For HHC‐P, all similar level efficacy. evidence suggests cannabimimetic effects tested exception acetates likely function pro‐drugs vivo.

Delta-8 THC (D8) is an intoxicating compound linked to poisonings and hospitalizations, no federal laws prohibit its sale minors. We examined retail D8 availability near public schools (n=231) in Fort Worth, Texas, where sales are minimally regulated. Between September-October 2021, trained data collectors collected from 1,178 retailers with alcohol, tobacco, and/or consumable hemp licenses. differences product for (within 0.5 miles) vs. not schools. There was a higher prevalence of selling (vs. near) (12.4% 8.2%; chi-square test 5.84, p<0.05). Of the 231 schools, 37.2% (n=86) had at least one retailer nearby. 86 nearby, cheapest products were vaping (40.7%) edibles (31.4%). school-level racialized socioeconomic inequities availability. A six-year-old boy presented to the pediatric emergency department following an accidental ingestion of a synthetic cannabinoid. The child ingested edible product containing tetrahydrocannabinol (THC) and with lethargy, altered mental status, increased muscle tone. airway was protected, patient breathing spontaneously. Initial assessment included thorough history provided by caregivers, which confirmed cannabinoid gummies approximately two hours prior presentation. An electrocardiogram demonstrated sinus tachycardia prolonged QTc. Toxicology screening presumed positive for cannabinoids. Therapeutic management focused on supportive care. admitted hospital discharged day no neurologic sequelae. caregivers were expectant counseled regarding safe storage use cannabis cannabis-derived products. This case report discusses growing incidence exposure in population while highlighting biochemistry, clinical presentation, therapeutic
