Background/Objectives: The aim of this study was to assess the unmet needs myelodysplastic neoplasm (MDS) patients and their caregivers, focusing on how these impact quality life (QoL) daily functioning. MDS predominantly affects older adults. It is often complicated by severe red blood cell transfusion-dependent anemia may require frequent hospital visits, conferring a substantial burden caregivers. Methods: A national survey conducted between June 2022 May 2023 in 46 hematology centers across Italy, involving 259 105 included validated QoL tools (QOL-E HM-PRO) measure disease treatments health-related symptoms. Results: Of surveyed, 42% were transfusion-dependent, with 45% reporting distress related travel, which significantly associated lower scores physical score 50.0 vs. 62.5, p < 0.001). Transfusion dependency led worse outcomes physical, emotional, social domains (HM-PRO Part 59.8 23.7, Anxiety affected 66% patients, while 56% reported feeling emotionally distressed. Patients requiring visits showed notably emotional 56.8 31.8, Conclusions: This identified for particularly addressing heavy transfusions visits. Both caregivers experienced QoL, highlighting urgent need that reduce dependency, improve patient outcomes, alleviate caregiver burden.
