Abstract Objective: Pumping studies in mothers of preterm infants are limited by self-reported pumping behaviors and non-objective measures pumped milk volume secretory activation (SA). Study Design: Non-randomized observational study first 14 days postpartum 29 infants. Smart pumps measured stored volume. Selective ion electrodes sodium sodium:potassium ratio to determine SA. Generalized estimating equations, cluster analyses multivariate regression were used. Results: SA was delayed (median 5.8 days) impermanent. Each additional daily increased odds within 2 48% (p=.01). High-intensity (N=17) had greater cumulative than low-intensity (N=12). variables showed changes the week, then plateaued. Conclusion: The week is critical for optimizing behaviors.
