BACKGROUND: In elderly individuals, excessive lactate levels in the brain may be associated with development of cognitive impairment after surgery, including delayed neurocognitive recovery (dNCR). Since origin this increased is unknown, here we assessed associations between metabolic pathways and postoperative dNCR. METHODS: This study included 43 patients (≥60 years old) who had surgery under general anesthesia. We also used a mouse model which 20-month-old mice were exposed to sevoflurane induce dNCR, while control 40% oxygen. Mice group anesthesia/surgery injected fructose or glucose intracerebroventricularly, metabolism inhibitor intraperitoneally. Barnes maze test Y measure function mice. Metabolomics was metabolites serum brains anesthesia/surgery. Isotope labeling flux analyze flow distribution specific pathways. RESULTS: Among patients, 17 developed showed significantly decreased 1-phosphate dNCR compared nondNCR (mean difference [×10 4 ] = –0.164 ± 0.070; P .024). The role of nutrition in shaping the immune system is a critical aspect maintaining overall health, particularly during vulnerable periods like early lactation dairy cows. Various nutrients, including glucose, glutamine (Gln), and ketones, have been found to impact cell function. Gln, versatile amino acid, serves as precursor for DNA RNA synthesis, NADPH production, energy generation, all crucial responses. Gln plays pivotal cytokine synthesis phagocytosis, macrophages monocytes. Glucose essential proliferation, survival, differentiation cells, with activated cells relying on it their primary metabolic fuel. Restricting glucose uptake phagocytic has shown impair function, which highlights importance availability when mounting an effective response. Ketones, especially β-hydroxybutyrate (BHBA), associated compromised function Elevated BHBA levels can hinder chemotaxis respiratory burst activity neutrophils, potentially increases risk infections mastitis. Additionally, may lymphocyte proliferation antibody secretion further compromise defenses. Non-esterified fatty acids (NEFAs) complex effects both stimulatory inhibitory observed. NEFAs phagocyte immunoglobulin (Ig)M secretion, production. type concentration circulation determine whether they enhance or suppress Energy balance these interactions, negative (NEB) exacerbating immunosuppression. Epidemiological studies linked metabolites characteristic NEB mastitis risk, suggests that multiple blood serve better indicators than individual markers.
