Maintenance of endothelial cell integrity is an important component human health and disease since the endothelium can perform various functions including regulation vascular tone, control hemostasis thrombosis, cellular adhesion, smooth muscle proliferation, inflammation. Endothelial dysfunction encompassed by complex pathophysiology that based on nitric oxide synthase uncoupling activation following stimulation from inflammatory mediators (molecular patterns, oxidized lipoproteins, cytokines). The downstream signaling via nuclear factor-κB leads to overexpression adhesion molecules, selectins, chemokines facilitate leukocyte rolling, transmigration subendothelial space. Moreover, oscillatory shear stress pro-inflammatory with increased monocyte apoptosis, effect dependent multiple pathways flow-sensitive microRNA regulation. role neutrophil extracellular traps NLRP3 inflammasome as mechanisms contributing has recently been unveiled under further investigation. Consequently, their activation, injured cells release enter a pro-thrombotic state through coagulation pathways, downregulation thrombomodulin, increase in platelet aggregation owing action von-Willebrand factor, ultimately promoting atherosclerosis progression.

Endothelium plays a fundamental role in the cardiovascular system, forming an interface between blood and adjacent tissues by regulating vascular tone through synthesis of nitric oxide, prostaglandins other relaxing factors. Endothelial dysfunction is characterized vasoconstriction, cell proliferation shifting toward proinflammatory prothrombic state. In hypertension endothelial may be involved initiation development inflammation, remodeling, atherosclerosis independently associated with increased risk. Different conditions such as impaired shear stress, inflammation oxidative activation renin angiotensin system have been described important pathophysiological mechanisms dysfunction. The release extracellular vesicles neighboring cells wall has emerged regulator function potential antihypertensive properties beneficial effects counteracting mediated organ damage. Furthermore, macrovesicles are emerging innovative therapeutic approach for protection, allowing delivery bioactive molecules, miRNA drugs interacting system. this review we summarize available evidence about implications diseases, focusing on its sequelae, strategies targeting endothelium aim to improve remodeling.

Abstract Aims Endothelial function is essential for cardiovascular health, and flow-mediated dilation (FMD) an established technique to measure it. This paper aims assess FMD values in apparently healthy individuals provides reference facilitate wider clinical use. Methods results In 1,579 (aged 18–76), fasted (data from 44 studies, 6 institutions, 22 operators) were normally distributed inversely univariately correlated with age, body mass index, glucose, cholesterol, blood pressure, brachial artery (BA) diameter. Significant multivariate predictors of age (−0.4%/decade), BMI (0.04%/kg/m2), smoking (−0.7%), BA diameter (−0.44%/mm) that together explained 19% the variability independent operator, institution or ultrasound machine. Individuals high tertile (>6.8%) younger, had smaller diameter, lower pressure cholesterol. low- intermediate fatal risk (SCORE), 26% 53% individuals, respectively, low (<5.4%). After adding data 385 patients stable coronary disease (CAD), ROC analysis (c = 0.841, P < 0.001) showed >6.5% excluded CAD (95% sensitivity; 60% specificity) <3.1% 95% specificity, 31% sensitivity). A meta-analysis meta-regression 82 trials (11 countries, n 3,509) using similar methodology despite considerable heterogeneity (I2 0.97) was on average 6.4% (95%CI: 6.2%, 6.7%) no significant differences between countries but a age-dependent decline (−0.3%/decade, R2 0.13). Conclusions We provide age-adapted frame intervals use as biomarker health. As degree vascular endothelial integrates environmental genetic factors classical CV factors, may more comprehensively classify without standard modifiable serve target prevention.
